Job Purpose

The Support Coordinator serves as a support to students with intellectual disabilities (ID) in the REACH Program. This includes, but is not limited to, case management, teaching, service coordination, counseling and the implementation of services and accommodations for students with ID.

Minimum Requirements

A master’s degree or equivalent experience in social work, counseling, special education or related field is required. Two years of experience in the field of intellectual disability (ID) is preferred. Social work licensure is preferred. Experience with implementing and facilitating educational groups is also preferred. Candidates with an equivalent combination of experience and/or education are encouraged to apply.

Required Knowledge, Skills and Abilities

Working knowledge of inclusive education for students with mild intellectual disabilities; excellent student service and problem solving skills; excellent oral and written communication skills, including proper documentation and protection of sensitive information; ability to effectively train others; strong interpersonal skills and experience working with diverse populations; knowledge of FERPA compliance, with ongoing attention to compliance; must be proficient and comfortable with use of computers / technology ; ability to independently manage caseload, while working collaboratively within a team structure; ability to adapt to the needs and goals of the office and attend to day to day operations as well as student crises that may arise; ability to evaluate students’ needs to create a comprehensive accommodation plan; must be a self-starter with the ability to work in a fast paced, team-oriented environment

Additional Comments Regarding Position

This position is a full-time position. The support coordinator will be part of a one week on-call rotation approximately every 6 weeks. The position requires communication with students during evacuations from the college (such as during a hurricane).

Activity

Facilitate a weekly group for seniors in the REACH Program to discuss class requirements, self-determination, goal setting, financial planning, and other obligations.

Ensure students maintain their independence and daily life skills. This includes, but is not limited to, personal care, living and working with others, healthy life choices, finances, and community engagement which are outlined in the students’ PATH Plan.

Guide students in integrating into their communities through community engagement, volunteerism and networking.

Assist students in their transition from college living to independent life.

Instruct REACH support sessions including Advanced Personal Finance and Now What?! Preparing for Life after College in preparation for graduation.

Communicates with and advises the career coordinator on best-suited internships for students.

Builds community partnerships and collaborates with the REACH team to ensure student success.

Activity

Review and interpret highly detailed and complex disability documentation in order to effectively determine and implement appropriate supports for students. Ensures security and confidentiality of disability documentation and information.

Assist faculty with the implementation and understanding of the modifications/ accommodations for each student while being respectful of the student’s FERPA rights and academic needs.

Update and maintain student records (e.g., Salesforce notes, schedules, EOY reports, midyear reviews, grades, etc.).

Activity

Oversee work with students who may need accommodations that are not available through the Center for Disability Services/SNAP. Communicates confidentially with program assistants, tutors, professors and other REACH personnel regarding students’ progress and struggles with ADL, academics, and social settings.

Guide students in-person and/or virtually for 6 months post-graduation as they begin integrating into their communities.
